Myo-inositol has become a popular supplement in conversations about PCOS and insulin resistance. Metformin, by contrast, is a prescription medication with decades of clinical use for type 2 diabetes and certain PCOS-related metabolic concerns.
It is tempting to ask which one is “better,” but that question is more complicated than a headline suggests.
What Is Myo-Inositol?
Inositols are compounds involved in cell signaling, including insulin signaling. Research has explored myo-inositol alone and in combination with D-chiro-inositol in people with PCOS, metabolic syndrome, gestational diabetes risk, and insulin resistance.
What Is Metformin?
Metformin is a prescription medicine that lowers liver glucose production and improves insulin sensitivity. It is widely studied and is used for type 2 diabetes and in selected people with PCOS. Gastrointestinal side effects are common, especially when starting treatment, but dosing and formulation can sometimes be adjusted by a clinician.
Does Research Prove Myo-Inositol Is Better?
Some reviews report improvements in insulin-related markers with inositol, and some trials compare inositol with metformin. However, study sizes, populations, doses, and outcomes vary. That is not the same as proving that myo-inositol should replace metformin for every person with insulin resistance or PCOS.
If you take metformin, do not stop or replace it with a supplement without discussing the change with your prescriber.
Questions to Ask Your Healthcare Professional

- Do I meet criteria for PCOS, prediabetes, or diabetes?
- Could inositol be appropriate alongside my current treatment?
- Are there medication, pregnancy, or fertility considerations?
